Full-Time | Competitive Pay |26 Weeks Contract with Potential of Conversion to Permanent

📍Location: Jacksonville, FL, 32206

The case manager will support 3 SNFs located within a 10-mile commute from Glynlea/Grove Park.

🕖 Shift: 8 hrs. days Shift, 8 am -5 pm (Mon to Fri)

📅 Scheduled Hours: 40 Hours a Week

Hourly Compensation: $45.00 - $50.00 Per Hour. (Based on experience)

Job Duties:

  • Ensure accurate and complete documentation.
  • Communicate and collaborate with the interdisciplinary care team.
  • Assist the provider/team with various care coordination activities.
  • Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits.
  • Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition.
  • Assist the provider/team with therapy coordination and participate in facility-based meetings.
  • Schedule and participate in family conferences, team meetings, and team case presentations
  • Coordination of training, tracking, and compliance with quality measures.
  • Assist in creating and conducting in-services for selected audiences
  • Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.

What We’re Looking For:

  • An active RN license for FL state or hold multi state compact license.
  • BLS from AHA
