Bilingual Virtual Behavioral Health Care Manager (LCSW, LPC, LMFT, PhD)

Remote within Houston, TX

The Behavioral Health Care Manager is a core member of the collaborative care team , including the patient’s primary care provider and psychiatric consultant, as well as the larger primary care team or medical team. The Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

  • Patient Care Support : Provide therapy and case management to assist in the mental and physical health care of patients assigned to your caseload. Collaborate closely with the patient's primary care provider and, when appropriate, other professionals to coordinate care effectively.
  • Patient Screening & Assessment : Refine Diagnosis, screen and assess patients using tools such as PHQ-9 and GAD-7. Facilitate patient engagement and ensure timely follow-up care.
  • Monitoring Treatment Progress : Systematically track treatment responses, monitoring patients for changes in clinical symptoms, side effects, or complications of treatment.
  • Medication Management Support : Support the management of psychotropic medications as prescribed by medical providers, focusing on treatment adherence, monitoring for side effects, and assessing the effectiveness of treatments.
  • Behavioral Interventions : Provide brief, evidence-based behavioral interventions, including techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other appropriate interventions based on patient needs.
  • Referrals for additional treatment options : Facilitate in-system or external referrals as clinically indicated.
  • Patient Follow-up & Documentation : Keep appropriate documentation in the EHR and registry including tracking patient follow-up and clinical outcomes.
  • Relapse Prevention & Discharge Planning : Develop and complete relapse prevention and self-management plans for patients who have met their treatment goals and are nearing discharge from the caseload.
  • Safety & Compliance : Ensure the safety of patients, staff, and visitors by adhering to all Memorial Hermann policies, procedures, and standards. Maintain compliance with budgetary specifications, including time management, supply management, productivity, and quality of service.
  • Professional Development : Promote professional growth and development by fulfilling requirements for mandatory/continuing education and skills competency. Contribute to department-based goals that support organizational success. Serve as a preceptor, mentor, and resource for less experienced staff members.
  • Other Duties : Perform other duties as assigned.

Minimum Qualifications

  • Bilingual in Spanish & English required for this position.

Education: Graduate of an accredited Mental Health Master’s Program (MSW, LPC, LMFT, PhD)

Licenses/Certifications: Current/Active licensure as an independent licensed mental health professional in the state of Texas required

  • Licensed Social Worker (LCSW preferred, LMSW considered with completed supervision hours)
  • Licensed Professional Counselor
  • Licensed Marriage and Family Therapist
  • Licensed Psychologist
