Job description
Seeking an experienced APP to manage both acute and chronic MEDICAL diagnosis and symptoms for patients with behavioral health. Must have expert communication, collaborative, and organization skills. This position requires clinicians to be an integral member of our interdisciplinary team, caring for patients with sensitive healthcare disorders.
Experience: Prefer a minimum of 3 years previous related acute and chronic healthcare management experience caring for both adolescents and adult patients.
Education / Training: Nurse Practitioners or Physician Assistant required. Graduation from either an accredited nurse practitioner or physician assistant program with corresponding board certification. California licensure, DEA, and BLS required.
Essential Duties and Responsibilities:
1. Must be competent in the management of acute and chronic medical diagnosis, symptom management and problem solving. The individual in this role will be the primary medical provider covering for an eating disorder and a behavioral health facility.
2. Must be able to work independently with a patient population that includes pediatric through adulthood. (You will have a supervising physician that will be available via phone for consultation as needed).
3. Perform a complete evaluation on new patients and consults to include: a history of present illness (to include location, severity, timing, modifying factors, quality, duration, context, and associated signs and symptoms), past medical / surgical history, review of systems with documentation of pertinent positives and negatives, family and social history, med reconciliation, allergies, comprehensive physical examination (P.E.), formulating appropriate diagnosis, and plan of care development & implementation reflective of appropriate medical decision making.
4. Performs focused history, chief complaint for all follow-up patient visits to include: review of diagnostic findings / objective data, medications, hemodynamic parameters, physical exam, assessment and plan.
5. Order and/or perform, collect, analyze, and integrate the findings of diagnostic tests and procedures to formulate a plan of care or a modification to the current management plan based on the patient's condition. Provides the appropriate patient education concerning the need, preparation, nature, and effect of diagnostic and therapeutic procedures.
6. Coordinate and communicate patient care needs with the patient / family, physician and/or other health professionals on an ongoing basis. Other professionals include nursing, therapists, psychiatry, dietitian, case managers, and other ancillary support team members.
7. Must have knowledge to appropriately code and submit bills for processing.
