Our client is seeking Licensed Practical Nurses to join their Home Health team serving Western Pennsylvania. This position offers an opportunity to provide compassionate, patient-centered care in the comfort of patients' homes while working within a collaborative and supportive environment.
Responsibilities include managing and delivering individualized patient care plans, coordinating care with the clinical team, supporting patients across Western PA, completing documentation accurately and on time, and effectively managing your schedule. The role requires proficiency with tablets, mobile devices, and EMR software.
Schedule: Monday through Friday, standard 8-hour days, with occasional weekend rotation (approximately one weekend per month).
Travel: Local travel required within assigned areas.
Required qualifications:
Active Licensed Practical Nurse (LPN) license in the State of Pennsylvania
Associate degree or higher in Nursing
Minimum of one year of experience using EMR systems such as TruChart
Valid driver's license and reliable transportation
Ability to manage local travel requirements
