Job Description

We are seeking a dedicated and compassionate Family Nurse Practitioner specializing in Senior Care to join our healthcare team. In this role, you will provide comprehensive primary and specialized care to elderly patients across various settings, including assisted living facilities, nursing homes, and outpatient clinics. Your expertise will support the health and well-being of seniors by delivering personalized assessments, managing chronic conditions, and coordinating multidisciplinary care plans. The ideal candidate will demonstrate a commitment to enhancing quality of life for older adults through evidence-based practices and empathetic patient engagement.

Duties

  • Conduct thorough physical examinations and health assessments for senior patients, including those with complex medical histories.
  • Develop and implement individualized care plans that address acute and chronic health issues, emphasizing geriatrics and memory care.
  • Manage medication administration, including injections, IV infusions, and botulinum toxin treatments where applicable.
  • Perform diagnostic evaluations utilizing EMR (Electronic Medical Records) systems such as Epic or eClinicalWorks; interpret lab results and imaging reports.
  • Provide triage services to determine urgency levels, coordinate referrals to specialists like cardiology or neurology, and facilitate hospital admissions when necessary.
  • Engage in health coaching to promote lifestyle modifications, nutrition counseling, and disease prevention strategies tailored for older adults.
  • Assist with procedures such as suturing, catheterization, spinal taps, and sterile processing techniques; ensure infection control compliance.
  • Document all patient interactions accurately in medical records while adhering to HIPAA regulations.
  • Participate in clinical research initiatives related to senior health or dementia care; contribute to quality improvement projects aligned with NCQA standards.
  • Collaborate with interdisciplinary teams—including social workers, physical therapists, and case managers—to optimize patient outcomes across inpatient, outpatient, and post-acute settings.

Requirements

  • Master’s degree in Nursing with certification as a Family Nurse Practitioner (FNP) from an accredited program.
  • Valid state licensure as a Nurse Practitioner; national certification preferred.
  • Demonstrated experience working with elderly populations in settings such as nursing homes, assisted living facilities, or home care environments.
  • Proficiency with EMR/EHR systems like Epic, Athenahealth, or Cerner; experience with telehealth platforms is advantageous.
  • Strong knowledge of geriatrics, dementia care—including Alzheimer's disease—and senior-specific pharmacology.
  • Familiarity with procedures such as IV insertion, venipuncture, suturing, catheterization, and basic life support (BLS).
  • Experience managing complex cases involving acute pain management, infectious disease care, hospice & palliative medicine.
  • Knowledge of CPT coding for billing purposes; understanding of Workers’ Compensation law is a plus.
  • Excellent communication skills with diverse patient populations; ability to collaborate effectively within multidisciplinary teams.
  • Critical care experience or ICU background is desirable; familiarity with Level I Trauma Center protocols enhances suitability.
