Position Summary:
Responsible for educating individuals with diabetes and their families through use of the Living with Diabetes DSMES (diabetes self‐management education and support) Program and one on one learning opportunities. Initiates appropriate measures to inform the public and the nursing staff of the availability of the program, identifies educational needs of diabetics and of those who care for diabetics, plans and implements programs to meet the identified needs. Oversees the day‐to‐day operations of the DSMES service and ensures that the national standards are always met and maintained.
Education, License & Cert:
1.Baccalaureate degree in nursing, dietetics, and/or education.
2. NYS licensed registered nurse and/or NYS certified registered dietitian; certified from the N.C.B.D.E. in diabetes education or able to sit for the exam within 12 months.
Experience:
1. Two or more years experience in the area of diabetes management strongly preferred.
2. Academic and/or experiential preparation in program management and in the care of people with chronic disease.
3. Demonstrated competence in teaching and in applying principles of teaching/learning processes.
4. Demonstrated competence in skills of communicating, leadership and creativity with groups. 5. Demonstrated ability and/or willingness to develop skills in teaching and in applying principles of the teaching/learning process with adult learners and child learners.
6. Experience in insulin pump therapy or willingness to learn all aspects of insulin pump therapy to become a certified pump trainer.
7. Microsoft Office computer skills required; to include Word, Access and Excel.
Essential Functions:
1. Educate individuals with diabetes and their families through use of the Living with Diabetes DSMES (diabetes self‐management education and support) Program and one on one learning opportunities.
2. Identifies educational needs of diabetics and of those who care for diabetics, plans and implements programs to meet the identified needs
3. Conducts follow‐up on all diabetic clients and implements continuing education plan when indicated.
4. Oversees the planning, implementation/development, scheduling, and evaluation of designated educational programs (including the DSMES service) for patients, families and staff caring for patients.
5. Initiates appropriate measures to inform the public and the nursing staff of the availability of the program.
6. Monitors and facilitates maintenance of DSMES team members qualification (CE credits, training, competency, licensures, and registrations).
7. Responsible for maintaining ADA recognition and participating in the evaluation of the DSMES service’s effectiveness by maintaining appropriate records and data for assessment and evaluation purposes
8. Serves as the chair of the Diabetes Advisory Committee, arranges/coordinates the activities of the Advisory Group, and provides input to members when appropriate.
9. Liaises between the DSMES team members, the Advisory Group, and other departments and administration.
10. Prepares reports using collected data and presents to the Advisory Group.
11. Recognizes the importance of keeping abreast of new information and materials available to the institution and to the diabetic consumer. Keeps current on A.D.A. standards of care.
12. Received a minimum of 15 hours annually of continuing education in diabetes and/or educational principles.
Other Duties:
1. Travel for this position is sometimes required between Guthrie entities
2. Participation in community and employee engagement activities is required.

The pay range for this position is $29.04-45.78.
