Registered Nurse (RN)

Case Manager – Program & Workflow Lead

$140,000/year | Full-Time | Hybrid – San Francisco Bay Area

Healthcare Recruiters International (HCRI) is seeking an experienced Registered Nurse (RN) for a neuro-focused organization that offers Principal Care Management (PCM) and Chronic Care Management (CCM) services . This position is perfect for an RN who enjoys developing workflows, establishing structure, and leading clinical processes, rather than just handling daily tasks.

This is a hybrid position serving patients with Alzheimer’s, Parkinson’s, dementia, movement disorders, and other chronic neurological conditions .

What You Will Lead

• Build and improve clinical workflows, care coordination systems, and documentation processes
• Create structure around LPN tasking, outreach cadence, and care plan workflows
• Develop SOPs, quality checks, and scalable clinical processes
• Partner across clinical, operations, and product teams to optimize system efficiency
• Lead quality assurance for PCM/CCM documentation and Medicare compliance

Key Responsibilities

• Supervise and mentor LPNs on patient outreach, tasking, and clinical documentation
• Complete and approve care plans, time-based activity logs, and PCM/CCM billing support
• Conduct patient check-ins, manage escalations, and support neurologist partnerships
• Maintain high documentation standards in digital health platforms and EMRs
• Participate in weekly team meetings and cross-functional workflow discussions

What We’re Looking For

• Active RN license in California
• Experience building workflows, programs, or systems (required)
• Background in care management, population health, or chronic care
• Ability to work independently and lead LPN-level staff
• Strong documentation and organizational skills
• Experience with chronic conditions, neurology, dementia, or Alzheimer’s is a plus
• Knowledge of PCM/CCM billing preferred

Compensation & Schedule

• $140,000/year base salary
• Full-time, hybrid schedule
