ResponsibilitiesThe Pharmacy Transitions of Care Technician, under the direct supervision of a pharmacist, will assist the care team through various phases of the patient’s care including but not limited to: 1) Gathering the best possible medication history that is then utilized by the care team for medication reconciliation, and 2) Facilitates the provision of medications at the time of discharge from the hospital.
