Job Title: Mobile Nurse Practitioner – Senior Living Rounding + Wound Care Paid Training

Company: Twin Cities Physicians

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Location: Greater Twin Cities Metro Area, MN

Schedule: Full-Time or Part-Time

What You’ll Do:

  • Travel to senior communities to provide patient care
  • Perform wellness checks, chronic care, and wound assessments
  • Coordinate care plans with nursing and family members
  • Chart in EMR and ensure regulatory documentation

Qualifications:

  • Licensed Nurse Practitioner (AGNP or CNP)
  • Experience with wound care preferred (certification paid)
  • Passion for elder care
  • Self-motivated with strong communication skills
