Joyful Companions Home Care is a non-medical home care agency serving Minneapolis/Hennepin County and the greater Twin Cities metropolitan area. We serve seniors with memory loss, and people with disabilities in their homes by offering companionship and homemaker duties. Our services offer a solution for seniors who are able and prefer to remain at home by receiving excellent companion care with love and grace. We are a Christian-based business providing compassionate home care services for older adults and their families. Joyful Companions Home Care is growing, and we are seeking an additional Social Worker to serve as a full-time Intake Home Care Coordinator. We are looking for candidates with a genuine heart for seniors to join our team of compassionate professionals in a supportive work environment. This person must have a passion for serving families in need, and dementia experience is highly recommended. The Home Care Coordinator is responsible for developing, planning, implementing, supervising, and monitoring social services for our companion care clients. The position requires flexible hours, a cheerful, friendly disposition, and a "can-do" attitude! The ideal candidate's personality will incorporate warmth, kindness, and compassion while encompassing skills that include intelligence, energetic self-starter, and motivated character with excellent follow-through and people skills in order to meet our clients' needs promptly. We look for confident experience in problem-solving, willingness to work on a team, and an ability to adjust to changing situations. Knowledge of community-based resources for seniors is preferred.

Responsibilities include, but are not limited to:

  • Coordination of home care services through phone intakes and client home visits
  • Care consultations and assessments with potential client families
  • Quality assurance visits with existing clients 
  • Attend care conferences 
  • Data entry to maintain up-to-date information in the Client Management system
  • Mentor field employees and schedulers if needed
  • Educate families on the aging process, home care resource referrals, and dementia care techniques
  • Effective oral and written communication skills for conducting and documenting care plans 
  • Excellent interpersonal skills among the office team and field contacts
  • Experienced in telephone and computer skills 
  • Knowledge of the healthcare field is preferred 
  • Connect quickly with people in a poised, convincing, and enthusiastic way
  • Work at a fast pace cooperatively with and through others in order to complete tasks
  • Miscellaneous administrative duties 

Position reports to office staff: Owner/Social Workers, and Operations Manager. 

Approximate start date: Immediately

Full Time weekday hours

Requirements: 

  • 5+ years in Senior Care - Dementia experience preferred
  • Bachelor's Degree in a human services-related field. MN Social Work License preferred
  • Possess Exceptional Communication Skills -written, oral, and interpersonal
  • Experienced Computer Skills 
  • Reliable Vehicle 
  • Current MN Driver's License 
  • Up-to-date Automobile Insurance 
  • Heart for working with older adults
