Job Title: Family Caregiver

Job Summary:
Guardian Home Care is seeking compassionate and dedicated individuals to join our team as Family Caregivers through Medicaid’s CFI (Choices for Independence) program. If you have a loved one who qualifies for Medicaid’s CFI program, you may be eligible to become their paid caregiver. As a Family Caregiver, you will provide essential, high-quality care to your family member, ensuring they can continue to live comfortably at home. Key Responsibilities:
· Assist with personal care and hygiene (bathing, dressing, grooming)
· Provide mobility support (help with walking, transferring, or using mobility aids)
· Prepare meals and assist with feeding if needed
· Remind your family member to take medications as directed
· Perform light housekeeping tasks (laundry, cleaning, organizing)
· Provide companionship and emotional support
· Document daily activities and report any changes in condition
Qualifications:
· Must be a family member of a Medicaid/CFI-eligible individual
· Must complete paid caregiver training provided by Guardian Home Care of NH
· Compassionate, patient, and committed to providing excellent care
· Reliable and able to follow a care plan
· Good communication and time management skills
· Ability to lift or assist with mobility support (as needed)
